Description

Position Summary: At Yellowstone Boys and Girls Ranch, we count on our Targeted Case Managers to support a trauma-informed environment while primarily providing targeted case management services to youth and families with social, emotional, and behavioral problems. The four core areas of targeted case management are assessment, development of service plans, referring activities, and monitoring of services.

They serve as a positive role model and mentor to the youth they serve. A Targeted Case Manager will have a heart and passion for working with youth who frequently come from hard places and have experienced complex trauma. The applicant must be mission and values-driven and possess a strong work ethic, reliability, and desire to be a positive role model in our youth and families' lives.

Daily and Monthly Responsibilities:

  1. Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, coordination of the various services, education regarding community resources, and referral of a youth/family to available services within the community.
  2. Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
  3. Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.
  4. Work with the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings.
  5. Complete, update, and maintain all necessary documentation within the guidelines set by COA, Montana ARM's, Contracts, and YBGR Policy and Procedures.
  6. Complete, submit, and verify accuracy of paperwork, including but not limited to progress notes, intake paperwork, etc., while maintaining Performance Quality Improvement (PQI) and financial viability of the program.
  7. Contribute to the strategic goals and outcomes of the agency as measured by the development and achievement of individual, departmental, and organizational goals and outcomes. Participate in Performance Quality Improvement (PQI) goals of the organization individually and as a team member.
  8. Deal constructively and helpfully with all other members of the organization.
  9. Fully support and model the mission and values of YBGR.
  10. Adhere to all policies, systems, safety requirements, and procedures of the organization.
  11. Take the initiative to identify problems and seek out solutions.
  12. Be a professional representative of the organization to those outside it.
  13. Participate and engage in monthly supervision/coaching meetings with the supervisor.
  14. Be aware of the cultural and socioeconomic characteristics of persons served.
  15. Participate in required training as appropriate to the position and beneficial to YBGR.
  16. Other duties as assigned

Qualifications


Required Skills and Qualifications

  1. A Bachelor’s degree in a mental health-related field, human services, social work, psychology, or related field of study is required.
    1. An equivalent combination of education and experience may be considered in lieu of a degree.
  2. 21 years of age or older
  3. Driver’s License to operate a passenger vehicle.
  4. Display regular, predictable, and reliable attendance at work
  5. Willingness and ability to obtain certification in First Aid/CPR and Violence Intervention Prevention.
  6. Successfully complete Child and Adolescent Service Intensity Instrument (CASII) training within 30 days of hire.
  7. Knowledge of the specific treatment/mental health needs of youth with emotional and psychiatric problems and their families
  8. Excellent written and verbal communication skills
  9. Knowledge of local community resources for youth and families.
  10. Good computer/technology skills. Able to quickly learn and use YBGR selected software solutions.
  11. Criminal / Child Abuse Clearance: Must pass criminal record and child abuse registry background check as required by the State of Montana.

Preferred Qualifications

  1. 2 (two) years of professional experience in case or treatment management is preferred.
  2. 2 (two) years of professional experience working with youth with serious emotional disturbances is preferred.

Physical/Environmental Demands & Requirements:

  1. Office Environment: Employees are protected from weather conditions or contaminants, but not necessarily occasional temperature changes.
  2. Must be able to remain in stationary position frequently.
  3. The person in this position needs to frequently move about inside the office to access file cabinets, office machinery, etc.
  4. Frequently operates a computer and other office productivity machinery such as copier and printer.
  5. The person in this position will consistently communicate with internal and external stakeholders and contacts. Must be able to communicate accurate information and accurately understand information communicated by those individuals.
