What are the responsibilities and job description for the Mobile Client Navigator position at Yolo Crisis Nursery?

The Mobile Client Navigator will assist the Family Violence Coordination Project by providing crisis intervention, risk assessment, safety planning, case management and navigation services to adult and minor victims of family violence and human trafficking in collaboration with Empower Yolo.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The following duties are typical of those performed by this job title; however, this position may perform other related duties.

  • Act as the on-site liaison providing crisis intervention, risk assessment, safety planning, case management and navigation services to adult and minor victims, as a part of the Family Violence Coordination Pilot Project, under the supervision of the Executive Director. Referrals will come via Empower Yolo, court, law enforcement, social services, and district attorney’s office and through walk-ins.
  • Provide facilitation, coordination, and case management services with the Crisis Nursery resources and individualized services for children and families, including all related documentation for services planned and provided.
  • Conduct risk assessments and develop safety plans by assessing clients for issues related to risk of further trauma; providing options in planning for personal safety on an ongoing basis; and developing individual case plans that prioritize needs and coordinate necessary services specific to each client.
  • Develop a wrap-around services plan for the family. Implement strength-based, family-centered services in the wraparound planning process with accompanying Crisis Nursery team with intakes and discharges.
  • Provide cross-functional team facilitation with the Crisis Nursery case manager to support and enhance the efforts of youth and their families/caregivers.
  • Provide information and referrals to community resources and natural supports in the creation of family driven, strengths-based service plans.
  • Participate with families as a complete unit with a focus on individual strengths, concerns, and needs ensuring parent voice, choice and access.
  • Work effectively with the referring contact by assessing the initial concerns, needs and expectations of service.
  • Work in collaboration with local law enforcement, private attorneys and multiple court and government systems, as necessary.
  • Transport families to and from the Crisis Nursery from remote locations.
  • Participate in team meetings, work groups, trainings and continuous quality improvement projects and processes.
  • Perform other tasks, as assigned.

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE

A degree in Human Development or Early Childhood Education and/or 3 years’ experience working with children and families in case management preferred.

CERTIFICATES, LICENSES, REGISTARTIONS

  • Certificate in CPR and First Aid
  • Valid California driver’s license

LANGUAGE SKILLS

  • Effective communication skills including writing, public speaking, active listening skills.
  • Conflict resolution skills
  • Bilingual in Spanish, preferred.

  • High tolerance and respect for individual survival strategies while promoting ever increasing healthier choices.

  • Ability to communicate the roles and relative perspectives of family team, community partners, inter- and intra-agency partners, coworkers, and management.
  • Must be able to elicit, appreciate, and incorporate divergent perspectives in safety action plans and strategies to help youth stabilize in a placement and connect them with a safe environment and family.
  • Experience developing creative methods to ensure individualized culturally sensitive services.
  • Experience with the management of scope of practice with attention to coordination of family support.
  • Experience with crisis intervention and peer counseling for survivors and their support networks.
  • Must be trained in trauma informed care.
  • Must be able to work independently and within a team.
  • Must have excellent organizational and project management skills.
  • Must have knowledge of office administration procedures and the use of and operation of standard office equipment.

COMPUTER SKILLS

Must be proficient in Windows, Microsoft Office, Outlook, database software and internet use.

Job Type: Full-time

Pay: $21.00 - $24.00 per hour
