Young Parents United, Inc. is a 501c3 nonprofit organization providing support services, advocacy and resource coordination designed to meet the unique needs of teen parents and their children, break destructive cycles and open the doors for growth and opportunities to achieve these successful outcomes for struggling teen parents. The organization fulfills its mission to help change the landscape for very young parents -- through a combination of case management, collaboration with other agencies, mentoring, child development and parenting skills training, educational programs, assistance with housing stability, high school education completion, college planning, employment assistance, homework help, childcare planning, a safety network, social connections, family activities and much more.

OVERVIEW OF POSITION
The Family Support Worker provides basic needs and education support to clients, which includes engagement in all aspects of YPU’s mission. A Family Support Worker is a frontline public health worker who performs a wide variety of duties at the paraprofessional level to engage, support, advocate and educate program participants in overall health and wellness. The FSW serves as a liaison or link between health and social services and the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, client support, assistance with basic needs, community education, informal counseling, social support and advocacy. Family Support Worker will perform a variety of duties related to client health and wellness to reduce health disparities including client engagement (both individual and group); assessment or health status and risk; referrals to community services; service coordination and care management; facilitation of client self-management skills around health status;parenting skills, life skills, data entry/Electronic documentation, scheduling of appointments and other related tasks.
EXAMPLES OF DUTIES
- Conduct outreach to identify and engage community members around health and wellness activities
- Assess health status and social determinants of health
- Attend and participate in team meetings
- Communicate with county and community-based agencies/professionals to assist with service coordination as needed
- Client care coordination and navigation - Arrange, log and document transportation for clients to and from YPU programs and essential appointments
- Enter client information into a computer system
- Assist clients with completion of forms and paperwork
- Schedule appointments for clients in clinic or other health/social services agency- when assistance is requested Case Manager
- Client Education: Provide clients with parenting education tools, educational information and other needed resources and materials
- Client Programs: Set up Conference Room and prepare snacks for programs, coordinate and provide rides for clients to and from programs, assist with nursery supervision while clients attend programs
- Assist with WIOA (work incentive program) supervision and delegating assignments
- Deliver emergency and other needed food and personal care supplies to clients as needed
- Assist with coordination of furniture moving and deliveries as needed (Deliveries are on a case by case basis only for special circumstances - cleared by Case Manager).
- Log all free store items, Uber Rides, Laundry assistance, food support in google doc forms and enter each in LGL notes
- Assist with Social Media Posts, Event Flyers and YPU Newsletters
GENERAL DAILY TASKS
- Remind and ensure all clients and guests to sign in daily
- Assist with childcare in YPU playroom / nursery during classes/programs and when parents are working with case manager or other tasks at YPU, assist with childcare on case by case basis for job interviews or critical appointments (less than 3 hours - Emergency release form required)
- Assist with client needs, as necessary
- Update and organize resource table and documents
- Organization of all YPU locations and storage spaces (Main office, Free Store, Food Pantry, Storage Room.
- Assist with doorbell and letting in clients and visitors .as well as ensuring all guests sign in
- Assist with taking out garbage and recyclables (This assignment is shared by all YPU staff)
- Cleaning and organizing the nursery (This assignment is shared by all YPU staff)
- Support the Administrative Assistant and Case Managers with projects, events, and assignments
- Necessary tasks delegated to you by the Executive Director, case managers or the Leadership Team
FREE STORE
- Restock office diapers, wipes, and personal care item bins from basement storage room
- Maintain a list of items low in stock or immediate needs, notify Administrative Assistant
- Pack-up free store orders as necessary
- Log all free store items provided to clients in google doc forms and enter each in LGL notes
- Organize and facilitate Winter clothing shopping event
- Organize and facilitate shopping for holiday needs (christmas, halloween)
FOOD PANTRY
- Check the Food Pantry clipboard and log any items listed in the Food Pantry Log Google Document
- Check and record the temperatures on all freezer and refrigerator storage
- Organize and maintain food pantry to be labeled and user friendly
- Notify Office Manager of low inventory
- Stock the 3rd floor and basement pantry when food orders arrive
- Label all food items with expiration date
- Dispose of expired items according to Regional Food Bank regulations
- Assignments delegated to you by YPU staff (making food boxes, creating recipe cards)

QUALIFICATIONS
A combination of experience, education and/or training which substantially demonstrates possession of the following knowledge, skills and abilities:
- Knowledge of local population demographics, assets and needs
- Knowledge of local community health disparities
- Ability to connect with and provide ongoing support in a behavior change process
- Demonstrated organizational and time management skills
- Effectively communicate in writing in order to document client information and progress
- Ability to follow instructions and incorporate feedback
- Communicate verbally with clients and co-workers in a clear and effective manner
- Knowledge of local health disparities and resources to address those disparities.
- Requirements and procedures for care coordination, data management, and Electronic Client Records.
