If this is you, apply with us TODAY!Department: Sales/OperationsReports To: DirectorsLocation: In-personPosition Summary:The Care Coordination Specialist is responsible for supporting members through comprehensive care planning, intake, and Enhanced Care Management (ECM) services. This role works collaboratively with internal teams and external partners to ensure timely, coordinated, and person-centered care.Key Responsibilities:Complete comprehensive intakes and assessments for new and existing membersDevelop, implement, and maintain individualized care plans in collaboration with members and care teamsProvide Enhanced Care Management (ECM) support in alignment with program requirementsCoordinate services across medical, behavioral health, and community-based providersConduct outreach and follow-up with members to ensure engagement and continuity of careDocument all activities accurately and timely in the electronic health record (EHR)Support service close-outs to ensure services are billable and compliantParticipate in weekly ECM and care coordination meetingsMaintain compliance with all regulatory, contractual, and organizational standardsCollaborate with Care Coordinators, CRCs, leadership, and external partners as neededQualifications:Bachelors degree required (healthcare, social work, public health, or related field preferred)Minimum of 1-2 years of experience in care coordination, case management, or a related healthcare roleKnowledge of ECM, care management models, and healthcare systems preferredStrong communication, organizational, and time management skillsAbility to manage multiple cases and meet deadlinesProficiency with EHR systems and Microsoft Office/Google SuiteBilingual skills a plus (if applicable)Skills & CompetenciesPerson-centered and trauma-informed approachStrong problem-solving and critical-thinking skillsAbility to work independently and as part of a teamAttention to detail and documentation accuracyProfessionalism and confidentialityWork Environment & Physical RequirementsAbility to sit, stand, and use a computer for extended periodsOccasional travel to community sites may be requiredCompensation & BenefitsSalary: 48,000-58,000Benefits: Medical, dental, vision, PTO, holidays, and 401K plan
