What are the responsibilities and job description for the Wraparound Facilitator position at Youth Advocate Programs, Inc.?
Status: Full-Time Salary FLSA Classification: Exempt
Summary of the Position: Wraparound Facilitator will work with families of children with serious emotional disturbances by facilitating a planning process that is family centered, community-oriented, strengths-based, and highly individualized. Services may be provided in the participant’s home or appropriate community settings. The Facilitator will deliver intensive case management targeted at increasing the youth’s natural supports and strengths, fostering resilience and recovery. The Wraparound facilitator is responsible for accurately documenting services, facilitating Wraparound Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority. All services are delivered in compliance with the West Virginia CSED Waiver Procedures Manual, West Virginia Medicaid, YAP Standards, and all state and federal regulations governing the delivery of service.
Qualifications/Requirements: Bachelors of Arts (BA) or Bachelors of Science (BS) in a human service field with two years post-college work experience in this field.
Wraparound Facilitators must provide a unique approach to partnering with families and youth with complex behavioral health needs.
Demonstrated and documented competency in the work to be performed.
Position requires reliable transportation, valid driver’s license, and current auto insurance coverage.
