Summary
The Wraparound facilitator works out of our Chico, CA office. This position facilitates a family-centered, strength-based, culturally sensitive, needs-driven planning process for creating individualized services and supports for children, youth, and their families. Ensure the provision of Intensive Care Coordination (ICC) that is responsible for facilitating assessment, care planning, and coordination of service, including urgent services, for children/youth who meet the Katie A. subclass criteria. The Wraparound Facilitator assures access to natural, professional, and community-based options, activities, and opportunities. This position includes coordinating services, facilitation, and documentation along with all other functions and duties that are assigned.

Education and/or Experience
- A Master’s Degree in Behavioral Sciences
- BA degree in health or human services-related field from accredited college or university and at least three (3) years of experience with ED children, adolescents, and families
- Possess a valid State of California Driver License in good standing
