We are seeking a dedicated and compassionate HSPRS Case Manager to join our team! The goal of the HSPRS, or Home Study/Post Release, is to assess the suitability of a placement for an unaccompanied child in the custody of the ORR with a potential sponsor and, when possible, mitigate risks before the child arrives in the home.
The ideal HSPRS Case Manager candidate will provide essential case management and supportive services to unaccompanied immigrant children released from federal custody. This role is critical in ensuring the safety, well-being, and successful integration of these vulnerable children into the community.
Key Responsibilities:

Gather and document client data in accordance with program reporting requirements, ensuring accuracy for future evaluations.

Maintain detailed records of measurable outcomes to support service delivery and justification for any termination of services.

Submit written reports on time, adhering to all deadlines set by the program to ensure compliance and transparency.

Provide immediate crisis intervention when necessary, including coordinating with Child Protective Services to ensure the safety and well-being of clients.

Maintain complete and updated electronic case files for each child using the designated Electronic Medical Record system, ensuring data integrity and accessibility.

Ensure all required reports are documented promptly to facilitate effective communication and service continuity.

Timely notify appropriate personnel, parents, and agencies regarding incident reports, medical/dental emergencies, and other Notifications of Concern.

Attend all required meetings and participate in both formal and informal professional training to enhance skills and knowledge.

Actively seek out and engage in professional development opportunities to improve service delivery and personal competency.

Report findings and updates to the Director of HS/PRS Services and Lead Case Manager regarding quarterly and annual reports, assisting with program assessments.

Attend all training mandated by YFT and respective governing entities, including maintaining current CPR/First Aid certification and Handle with Care certification.

Complete annual TB tests and screenings as required to ensure compliance with health standards for service providers.

Perform other duties as assigned by the CEO, Vice President of Residential Services, or supervisor to support program goals and operations.
Qualifications
Qualifications:

Fluency in Spanish is required.

Bachelor’s degree in Social Work or a relevant field, or equivalent experience.

At least 1 year of experience in child welfare programs.

Knowledge of community resources and the ability to connect families with appropriate services.

Proficiency in Microsoft Office applications.

Ability to travel up to 70% of the time.

Valid Driver’s License required.

Must be able to pass an FBI background check and a CA/N check for all states of residence in the past five years.

Preferred: Knowledge and experience working with refugee or immigrant children, and cross-cultural or international experience.
