RN Private Duty Home Care

We are seeking a compassionate and skilled nurse who enjoys Private Duty Home Care and Care Management. You will work with an amazing team of professionals.

The position will focus on integrating disease management and lifestyle interventions into the care planning process. Working with physicians in managing more highly skilled home care needs.

Duties include:

• Conducting admissions
• Supervise caregivers
• Teach clients how to self-manage their chronic conditions
• Provide skilled nursing services with a doctor’s order
• Home visits to ensure compliance with treatment plans
• Care coordination and communication with multiple physicians
• Communicate with family members
• Community education
• And much more.

This position will work closely with the CEO in the delivery of care management.

• Licensed Registered Nurse
• Experience in home care preferred
• Experience in Care Management – will train
• Computer skills
• Requires excellent communication skills
