What are the responsibilities and job description for the Resident Services Case Manager position at YWCA of Brooklyn?

The Resident Services Case Manager is responsible for providing a comprehensive range of services to empower the residents of the YWCA Brooklyn, most of who are survivors of gender-based violence and other complex traumas. We use a strengths-based, client-centered approach to trauma-informed programming. Our team centers dignity, respect, and equity in all our work. The Resident Services Case Manager is an integral member of the YWCA Housing Department, helping to facilitate the goal of ensuring 100% of our residents remain stably housed through individual case management, leveraging community partnerships, providing appropriate referrals, implementing trauma-informed programming, participating in community collaborations, and advocating on behalf of our residents. The Resident Services Case Manager reports to the Director of Resident Services and works collaboratively with her to ensure the most vulnerable residents are accessing the resources they need to remain and thrive at YWCA Brooklyn.

Responsibilities:

  • Assess resident’s needs and develop client-centered care plans to connect and refer them to community-based services and programs including, but not limited to: medical and mental health services, social services, and substance abuse treatment programs
  • Liaise with community partners to provide consistent client advocacy to assure needs are being met once referrals and connections are made
  • Screen residents for eligibility for entitlements and benefits such as SNAP, Medicaid, housing subsidies (such as SCRIE/DRIE, LINC, Section 8, SEPS, EPVA, and CityFEPS), public assistance, HEAP, and others, and work with them individually to ensure applications and renewals are submitted in a timely fashion to ensure survivors’ consistent, stable housing and ability to live independently
  • Work collaboratively with the Resident Services team to resolve resident issues and provide crisis intervention
  • Develop and coordinate programming onsite, and connect resident survivors of DV, to economic empowerment initiatives; non-traditional therapeutic initiatives including arts, literature, and music; health and wellness activities; and programming that builds community, reduces social isolation and fosters mutual aid
  • Maintain proper documentation in the form of case notes, demographic data, and reports
  • Perform other duties as needed
  • Increase participation in group programming for residents
  • Design and facilitate groups onsite aimed at increasing resident participation and socialization; these groups include but are not limited to support, psycho-educational, and therapeutic in cooperation with community partners and consultants.
  • Obtain resident feedback regarding current and future programming to determine gaps through the use of resident surveys
  • Develop a monthly resident events calendar with seminars, workshops, and community activities

Qualifications:

  • Bachelor's Degree in social work or a related field required
  • Minimum of 2 years’ experience providing person-centered case management
  • Experience providing trauma-informed services to diverse populations
  • Capacity to multitask and work effectively in a fast-paced environment
  • Self-starter who can work independently and collaboratively in a group, is team-oriented, has a growth mindset and is willing to do what it takes to get the work done
  • Meticulous level of organization and attention to detail
  • Strong written, verbal, and interpersonal skills
  • Proficient in Microsoft Suite (Word, Excel, SharePoint, PowerPoint) and experience with data and reporting software, Apricot preferred
  • Bilingual in Spanish preferred, but not required

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
