Zepf Center has been serving the Lucas County community for over 40 years. We are the leading provider of behavioral health and vocational services to youth and adults with severe and persistent mental illness in the Northwest Ohio area. Services include child & adolescent psychiatric, medical, substance abuse, residential, and therapy services, as well as career development and wellness services. Zepf Center is also partnered with NHA to provide medical services to patients making access to care easier for them.

We are currently seeking a full-time Youth Community Based Case Manager. The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and to ensure their needs are addressed. The Integrated Care Specialist assists clients with the development of short and long-term goals and acquisition of community resources. Services will be provided with respect and in accordance with client’s rights.
The person in this position participates in development and coordination of the comprehensive care coordination and treatment plan, advocates for persons on their caseload, provides for assessment, linkage and referral, coordinates crisis intervention services and assists in the management of psychiatric and physical health symptoms. Levels of support required by people on caseload may vary greatly.
WORKING CONDITIONS
This job is a fast paced position that include a high volume of individual and family contact, collaboration with various community agencies, as well as computer entry and tracking and requires cultural/racial diversity and sensitivity. Employee must be able to maintain a patient engagement standard.
Employee's work is conducted in offices, participant's homes, and other community sites. Employee has contact with a wide range of people including some exhibiting anger and frustration under stressful conditions. Participants may be drug or alcohol-affected and in need of treatment. The employee will be exposed to and need to deal with a range of emotionally-charged issues in a compassionate, trauma-informed manner.
Must have a valid driver's license and acceptable driving record.
Requirements
- High School Diploma or GED required with Three years of mental health experience
- Preference will be given to those with a Bachelor's degree in social work or a related field.
- Must have valid drivers license and be able to provide proof of auto insurance
