Case Manager - Home Care/Hospice Jobs

Coordinates the overall care plan for home health/hospice patients. Evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Identifies patient care issues and makes recommendations.
