Home Care Case Manager Jobs

The Home Care Case Manager evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Coordinates the overall care plan for home health/hospice patients. Being a Home Care Case Manager monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers.
