The Patient Care/Case Management Nurse Manager evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Coordinates the overall care plan for patients from admission to discharge. Being a Patient Care/Case Management Nurse Manager monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Collaborates with physicians and other medical staff members to facilitate the progression of care.
