Job Purpose Summary:
- The FT Clinical Therapist provides clinical services to the clients of Harmony Bay Wellness.
- Interface with the Clinical Supervisor and other program staff in decision-making processes involving clients and relay all findings during clinical staffing.
Responsibilities Include:
- Assess the counseling needs of the individual/couple/family including appropriate level of care placement
- Assess and diagnose client for mental health/ substance use diagnoses using DSM and/or ASAM criteria
- Prepare individualized treatment plan(s) and provide mental health counseling and co-occurring portions of the client treatment
- Development of the client discharge/continuum of care plan to ensure movement to the appropriate level(s)of care
- Maintain positive and confidential rapport with client
- Document and maintain all counseling and education services, assessments, reassessments, referrals, and follow-up in the client’s clinical record (within 48 hours of occurrence) using EHR
- Provide coordination of support services for the client/family and record the services provided in the client’s clinical record following HIPAA laws and regulations
- Contact and coordinate with referral sources, (i.e. legal, school, employer, etc.)
- Facilitate needed interventions for crisis-oriented issues utilizing a multi-disciplinary team approach for assigned clients
- Contribute to practice on related tasks as needed
Goals For Success
- Clinical Therapists’ performance will be evaluated on several competencies including but not limited to:
- Completion of assessment tools within appropriate timeframe
- 70% of scheduled appointments completed weekly
- Average rating of 4.3 of 5 or higher on client satisfaction surveys
- Documentation completed within 48 hours of event occurrence
- Chart audit scores at or above 3.5% of 5% each quarter
- Appropriately documents higher level of care recommendations and any concerns related to high-risk reporting
Job Type: Full-time
Required education: Master’s Degree and active NJ License (LSW, LCSW, LAC, LPC, LAMFT, LMFT)
Required experience: 2-4 years’ experience working as a clinician
