Job Summary:

• Provide professional nursing activities as identified and planned by the R.N. Case Manager in the Patient Care Plan to maximize function within the home restoring health or assisting a peaceful death.

Key Responsibilities:

• Implement individualized patient care as initiated by the R.N
• Collaborate with interdisciplinary teams to develop and implement comprehensive patient care plans
• Conduct thorough assessments of patients' physical, emotional, and social needs
• Develop and implement effective care plans that meet the unique needs of each patient
• Provide ongoing education and support to patients and their families to promote healthy behaviors and outcomes

Requirements:

• Graduate of an approved school of nursing
• At least one (1) year of nursing experience in a hospital setting or community setting involving direct nursing care
• Current license to practice as a professional nurse in the State of Kansas and Missouri
• Active American Heart Association BLS
• Valid Drivers license

PREFERRED QUALIFICATIONS:

• Previous home health experience

Salary : $65,000 - $85,000
