About this Role
- Coordinates and directs the care of a caseload of home patients when the primary skill needed is nursing.
- Provides comprehensive assessment, planning, implementation, and evaluation for that caseload as the primary nurse.
- Assesses physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, cognitive status, and discharge planning needs of the home care patient.

Responsibilities
- Evaluates the patient for appropriateness of home health and develops the home care plan in conjunction with the physician.
- Edits and revises plans of care based on changes in the patient's condition or as needed.
- Ensures appropriate referrals to other services are made and interdisciplinary conferencing takes place regularly.
- Accurately and timely documents assessments and interventions.
