DescriptionDescriptionA SHC RN assess clients for services, determine clinical eligibility for programs and develop care plans. Our primary goal is to support individuals who receive services in the community so they can continue living in their own home. Duties include data entry into computer database, communication and consultation with family members, other Aging Services of North Central Massachusetts staff and community professionals.RequirementsROLE AND RESPONSIBILITIESConducts in-home clinical assessments for participants enrolled in the State Home Care ProgramAssess appropriateness for personal care services and create/maintain personal care plans in accordance with Executive Office of Elder Affairs (EOEA) guidelines.Conduct in-home safety assessments to determine appropriate level of care and make recommendations to reduce hazards in the homeDocument interactions and assessments in accordance with EOEA guidelinesConduct eligibility assessments for additional programs as needed Participate in interdisciplinary case conferences and foster ongoing communication with case management, protective services, and other ASNCM disciplines as appropriateParticipate in supervision with ASNCM Clinical Nurse Director for periodic reviews of completed workFosters an environment that facilitates professional collaboration among inter and interdisciplinary colleagues.Builds and maintains collegial environment that supports positive group processes.Other related duties as assigned.QUALIFICATIONS AND EDUCATION REQUIREMENTSGraduate of an accredited school of nursing, current MA nursing licenseMinimum three (3) years nursing experience required; community experience preferred.Excellent computer, organizational, interpersonal, written, and verbal communication skills essential  EXPERIENCE And interest in the elderly population and/or community health.
