Job Summary:
We are seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join our team. The LPN will play a crucial role in supporting patient care by working collaboratively with physicians, registered nurses, and other healthcare professionals to ensure the best outcomes for our patients.
Key Responsibilities:
- Provide direct patient care, including monitoring vital signs, administering medications, and assisting with procedures.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Document and maintain accurate patient records and communicate updates to the team.

Qualifications:
- Current and valid Licensed Practical Nurse (LPN) license in the State of Illinois.
- Graduate of an accredited LPN program.

- Strong communication, organizational, and interpersonal skills.
