Collaborate with the health care team to assess, plan, implement, monitor and evaluate options and services to enable the progression of patients across the care continuum.
Responsibilities:
- Maintain documentation
- Provide information regarding denials and approvals
- Provide education to patients, physicians, multidisciplinary team & the community
- Coordinate the case management process
- Coordinate the discharge planning process
- Perform utilization review duties
- Support PHIP
Requirements:
- Graduate of accredited school of nursing.
- Two years of recent nursing experience in an acute setting.
- RN License in the state of Illinois.
- AHA CPR certification
Salary : $35 - $55
