What are the responsibilities and job description for the Licensed Clinical Social Worker - Wellness Center position at ALABAMA REGIONAL MEDICAL SERVICES?

SUMMARY:

Provides psychosocial care for enrolled patients and families. Conducts bio-psychosocial assessments; and provides intervention and treatment modalities as assessed and determined.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Receive referrals from psychiatric team, primary care providers, and external community-based partners.

2. Conduct assessments for Mental Health and Substance Abuse challenges.

3. Employs a range of clinical interventions such as individual, group or family counseling.

4. Provides caregiver/family counseling/support to promote family cohesiveness, care to patients and prepare families for end of life.

5. Advocates on behalf of patients and families in addressing social determinants of health and wellness, including facilitating access to community-based services and needed resources, and refer patients to internal and external providers, as necessary.

6. Develops comprehensive bio-psychosocial assessments in accordance with age appropriate, cultural needs and concerns.

7. Provides mandated assessments when abuse is suspected and safety assessment when domestic violence is reported. Files reports as indicated.

8. Identifies patient’s psychological, financial, legal, psychiatric or substance use that affect patient care management, collaborates with the team to facilitate patient and family care planning.

9. Facilitates the appropriate and efficient use of hospital and community resources.

10. Participates in formal and informal clinical case reviews, clinical supervision, educational seminars and research projects.

11. Participates in quality assessment /improvement activities.

12. Assesses the patient/family learning needs, style and readiness. Engages and educates patients/families based on treatment plan, identifies barriers to care, recognizes diversity issues and learning styles.

13. Completes timely clinical documentation including the psychosocial assessments, progress notes, and billing compliance where applicable.

14. Adheres to and fosters compliance with NASW Code of Ethics, departmental and clinical, quality, compliance and safety standards, and ARMS’ policies, and procedures.

15. Ensures patients and supporting family members, guardians and care givers receive consistent information regarding all aspects of care where patient releases are authorized.

16. Attend required one-on one supervision sessions with supervisor, departmental staff meetings, and organizational-wide town hall meetings.

17. Perform other duties as assigned.

QUALIFICATION REQUIREMENTS:

EDUCATION AND EXPERIENCE:

Master of Social Work Degree from an accredited program required

Current Licensed Clinical Social Worker (LICSW) required

3-5 years of Clinical Social Work experience

Medicine Assisted Therapy (MAT) Certification and experience, preferred

Medication for Opioid Use Disorder (MOUD) Certification and experience, preferred

Bilingual, a plus

OTHER SKILLS AND ABILITIES:

Must have excellent interpersonal and communication skills.

Must have excellent computer and legible writing skills.

Knowledge of social service agencies, state regulations, and professional board standards as it relates to patient treatment, patient rights, and patient confidentiality.
