Job Description Summary


Provides a broad range of case management, therapeutic and psychiatric social work services to individuals with a mental health or co-occurring disorder.

  • Provide individual and group therapy to persons served with Mental Health or Co-Occurring disorders.
  • Provide case management services in consumers’ residences and at program.
  • Conduct intakes and submit recommendations for suggested treatment.
  • Ensure that persons served receive regular assessments and that services are provided in person centered approach.
  • Maintain a case load that is audit ready at all times.
  • Maintain accurate and sufficient documentation to permit successful entering into Electronic Medical Records (EMR) and allow for timely billing.
  • Write, maintain and submit accurate, complete and timely documentation (daily progress notes, consumer records, vehicle records, accident reports, incident reports) to ensure quality, service compliance with the IPOS and with funders.
  • Keep abreast of all required trainings including CPR, First Aide, Recipient Rights, and Person-Centered Planning.
  • Comply with Corporate Compliance Standards.
  • Facilitate crisis intervention, conflict resolution and stress management.
  • Respond immediately to emergencies.
  • Make referrals to supportive services.
  • Collaborate with other persons/groups including families, staff and community service providers.
  • Demonstrate strong analytical and problem-solving skills.
  • Demonstrate high level of interpersonal skills necessary to provide mental health services to consumers and their identified natural support systems.
  • Manage multiple tasks and time management.
  • Demonstrate knowledge of principles and processes for providing customer and personal services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Perform other related duties as required.


Physical Demands/Work Conditions


  • Physical, mental and emotional condition adequate for performance of the work.
  • May require evening and weekend working hours in addition to assignments in the community.
  • Ability to travel in the tri-county and other areas.
  • May require reliable transportation to carry out job duties.
  • Ability to walk, bend, stand, sit, and lift up to 35 pounds or less without assistance, drive, stretch/reach, and hand/finger dexterity.
  • Flexibility required.


Education/Experience/Credentials

Education

Required

  • Master’s degree in social work or counseling that is Medicaid billable from an accredited school.

Experience

Required

  • Moderate use of MS Word, MS Excel, MS Outlook, and Internet navigation.

Preferred

  • One year of experience in Community Mental Health including co-occurring disorders.
  • One year of experience in a non-profit Agency.

Credentials

Required

  • Limited License Master Social Workers (LLMSW) or Limited License Professional Counselor (LLPC).

Preferred

  • License Master Social Worker (LMSW) or License Professional Counselor (LPC).
  • CADC or CAADC

  • Demonstrated interpersonal communication skills to develop and maintain the confidence and cooperation of others; and to collaborate with staff, clients and other related groups on complex problems in order to take appropriate/effective actions and achieve results.
  • Ability to be flexible with changing and/or unpredictable work settings and assignments, work independently with minimal direction and to respond to written and oral direction.
  • Ensures compliance with funder, CARF, legal and AWBS requirements, standards and ethical practices.
  • Ensures quality and customer focused services.
  • Maintains confidentiality on behalf of consumers, staff and the Agency.
