Job Description Summary
Provides a broad range of case management, therapeutic and psychiatric social work services to individuals with a mental health or co-occurring disorder.

- Provide individual and group therapy to persons served with Mental Health or Co-Occurring disorders.
- Provide case management services in consumers’ residences and at program.
- Conduct intakes and submit recommendations for suggested treatment.
- Ensure that persons served receive regular assessments and that services are provided in person centered approach.
- Maintain a case load that is audit ready at all times.
- Maintain accurate and sufficient documentation to permit successful entering into Electronic Medical Records (EMR) and allow for timely billing.
- Write, maintain and submit accurate, complete and timely documentation (daily progress notes, consumer records, vehicle records, accident reports, incident reports) to ensure quality, service compliance with the IPOS and with funders.
- Keep abreast of all required trainings including CPR, First Aide, Recipient Rights, and Person-Centered Planning.
- Comply with Corporate Compliance Standards.
- Facilitate crisis intervention, conflict resolution and stress management.
- Respond immediately to emergencies.
- Make referrals to supportive services.
- Collaborate with other persons/groups including families, staff and community service providers.
- Demonstrate strong analytical and problem-solving skills.
- Demonstrate high level of interpersonal skills necessary to provide mental health services to consumers and their identified natural support systems.
- Manage multiple tasks and time management.
- Demonstrate knowledge of principles and processes for providing customer and personal services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
- Perform other related duties as required.
Physical Demands/Work Conditions
- Physical, mental and emotional condition adequate for performance of the work.
- May require evening and weekend working hours in addition to assignments in the community.
- Ability to travel in the tri-county and other areas.
- May require reliable transportation to carry out job duties.
- Ability to walk, bend, stand, sit, and lift up to 35 pounds or less without assistance, drive, stretch/reach, and hand/finger dexterity.

Education/Experience/Credentials
Education
Required
- Master’s degree in social work or counseling that is Medicaid billable from an accredited school.
Experience
Required
- Moderate use of MS Word, MS Excel, MS Outlook, and Internet navigation.
Preferred
- One year of experience in Community Mental Health including co-occurring disorders.
- One year of experience in a non-profit Agency.
Credentials
Required
- Limited License Master Social Workers (LLMSW) or Limited License Professional Counselor (LLPC).
Preferred
- License Master Social Worker (LMSW) or License Professional Counselor (LPC).
- CADC or CAADC
