We provide services to all age groups. We support over 800 children annually in our Early Intervention Program and 1500 adults in our Family Support Services. Education/Experience
BA/BS preferred. HS required. At least 1 year of experience as a service coordinator preferably with the DD Waiver.
SKILLS
Ability to learn computer applications with efficiency and accuracy, knowledge of community resources, and intermediate to exceptional verbal and written communication skills. Bilingual in Spanish and English preferred. Ability to initiate and complete work assignments independently with supervision, follow through with multiple tasks, directions and responsibilities.
Position Essential Job Functions
- Initiates and Monitors Respite Services:
- Completes initial referrals
- Schedules and completes intake interviews in order to obtain information necessary for service delivery
- Develops nurturing relationships with participant receiving services, in order to facilitate desired outcomes and/or meet needs
- Completes matching process to obtain provider(s) for the participant
- Ensures the family’s participation in completion of Individual Specific training for providers
- Partners with families as the experts on services for their family member
- Develops Action Steps and Outcomes and Semi-Annual Reports, as necessary
- Works with families, program participants and community resources to promote independence of the participant
- Gathers and communicates family input regarding quality of services
- Informs families of all services provided by Alta Mira
- Responds to family’s and/or participant’s need for changes in services or program
- Discharges participants when exiting program
- Monitors Respite Providers:
- Contacts Provider and schedules initial meeting to review Provider Agreement and other documentation necessary to provide services
- Completes home checks to ensure safety if services are to be provided in Provider home
- Disseminates information from initial meeting with the family, to appropriate individuals
- Ensures Providers receive information regarding participant prior to providing services
- Ensures Providers complete all Individual Specific training as outlined in the participant’s Individual Service Plan
- Completes Assisting with Medication Delivery coaching when necessary
- Follows through with any additional questions once Provider begins services
- Monitors Providers use of Electronic Visit Verification (EVV) & ensures compliance with the federal mandate
- Discharges Providers when exiting program
- Caseload Documentation Requirements:
- Obtains appropriate documentation from family initially and on an on-going basis
- Determines and monitors eligibility of State General Funded participants
- Obtains appropriate documentation from Case Managers and other team members initially and on an on-going basis
- Enters documents received in program database
- Files all appropriate participant information in participant file as outlined in service standards
- Completes and files all contacts made with family and participant including contact notes and ISP/IDT meeting documentation
- Forwards appropriate documentation to Respite Home when necessary
- Completes Quality Assurance reviews and works to obtain missing information from files
- Coordination with Interdisciplinary Team Members:
- Communicates concerns and needs to appropriate team members
- Coordinates Individual Specific Training between therapists and providers
- Ensures all IST is documented and filed in participant’s file
- Attends all ISP and IDT meetings for the participant in order to collaborate with all team members
- Ensures all documentation is received from team members to increase communication
- Communicates with participant’s family on a regular basis to address needs or concerns
- Policies and Procedures:
- Implements Developmental Disabilities Supports Division Service Standards for State General Funds and Medicaid Waiver participants
- Implements Family Supports Services internal policies and procedures for Respite Services
- Resource:
- Acts as a resource for participants and other community providers; including providing information about agency and state policies and procedures
- Assists participants in locating and accessing needed services within Alta Mira and/or from other agencies
- Participates in various fairs and events to promote and educate the community about services
- Crisis/Emergency Support:
- Performs on-call duties, as assigned
- Makes referrals to community resources when participants are in crisis
