Position: Case Manager Clinician

Department: Clinical

Education: A minimum of a Bachelor’s degree in a human service related field, e.g. counseling, social work. License as an LSW or LPC preferred.

Experience: 4-5 years of specialized training and experience closely associated with Community Psychiatric Supportive Treatment services shall be considered in the event a candidate does not possess a bachelor’s degree.

Qualifications: ACase Manager Clinician must possess formal education and/ or specialized training and knowledge of behavioral health issues and symptoms of mental illness commonly seen in the severely mentally disabled adult population. He/ she must possess knowledge and practical skills to assess consumer’s clinical and psychosocial needs, which are in turn used to develop appropriate service plans and interventions. Understanding of appropriate therapeutic interventions and use of clinical tools such as medications is needed. A Case Manager Clinician must demonstrate strong interpersonal communication and mediation skills with consumers, interdisciplinary team members as well as those in the community associated with their work, i.e., family, guardians, and other community agencies and entities. He/ she must build and demonstrate self-confidence in his/ her role within the agency and community. The ability to complete competent documentation of clinical information as well as strong general written and verbal communication is essential. A Case Manager Clinician must be able to work in an environment that requires flexibility, adaptability, multitasking, independent thinking, organizational skills, and time management. He / she must be able to respect the diversity/ individuality of others while also having the ability to set and maintain personal/ professional boundaries. He/ she shall have the ability to work with people in a professionally caring, respectful and ethical manner, while providing residents with therapeutic behavioral services (TBS), psychosocial rehabilitation (PSR), and case management services. Must possess an Ohio Driver’s license, have access to a car and be willing to transport clients. Proficient in Microsoft Office products (Word and Excel).

Immediate Supervisor: Case Manager Coordinator

Subordinates: None.

Position Summary: Case Manager Clinicians provide rehabilitative services to consumers intended to maximize the reduction of symptoms of mental illness in order to restore the consumer’s functioning to the highest level possible. Case Managers supports the consumer’s ability to take responsibility for managing his/her mental illness and achieving and maintaining his/her rehabilitative and/or recovery goals. Case Management staff may perform their work independently and/or as a part of a multidisciplinary team of professionals and trained others who are community based and mobile. On behalf of the consumer, Case Management Services may be provided to his/ her parents, guardian, family members and/or significant others. The service may be delivered in a variety of locations based upon the natural environment(s) of the consumer, i.e., home and community locations, in order to facilitate the consumer receiving the service, improved community functioning.

Essential Duties and Responsibilities:

1. Use specialized knowledge and skills to assess the consumer’s needs, including psychiatric, physical health, entitlement benefits, wellness, support system, and community resources, e.g., the need for housing, vocational assistance, income support, transportation, etc., in order to incorporate those needs and accompanying rehabilitative services and activities in the individual service plan (ISP).

2. Assist the consumer to acquire psychiatric symptom self-monitoring and management skills so that the consumer learns to identify and minimize the negative effects of the mental illness that interfere with his/her daily functioning.

3. Provide advocacy and outreach when the consumer’s mental illness prevents him/her from doing this for him/ herself.

4. Coordinate linkages to needed community services, support systems and resources. In addition, when the consumer’s mental illness impedes his/her ability to access these services him/herself, the service Clinician shall assist the consumer in accessing needed community services, support systems and resources.

5. Assist the consumer to develop the skills to access needed services, support systems and resources for him/her.

6. Coordinate and implement a consumer’s (Individualized Service Plan) ISP, including ensuring that the ISP reflects the most current interventions necessary to address the consumer’s mental health needs and symptoms of his/her mental illness. Ensure engagement of the consumer in the development of the plan while focusing on the recovery model.

7. Coordinate other services and Clinicians identified in the ISP to ensure that the ISP is being implemented as written.

8. Monitor a consumer’s progress in achieving goals and objectives/anticipated outcomes as documented on the ISP. Ensure complete and timely documentation of all interventions and work with the consumers.

9. Monitor the consumer’s status to identify when a change in mental illness symptoms indicates the need for a clinical review of the consumer’s mental health assessment and ISP.

10. Coordinate crisis assessment by emergency services clinicians and/or assist in crisis management and stabilization. Provide support and de-escalation in crisis situations, including working with the consumer, family, guardian and/or significant others as appropriate, to develop a crisis management and contingency plan.

11. Provide individualized, restorative interventions and training to improve interpersonal, community integration, and independent living skills when the consumer’s mental illness impacts his/her ability to function in and adapt to home, school, and work and community environments.

12. Provide training and skills development to consumers that may address socialization, communication, interpersonal relationships, and problem solving/conflict resolution, stress management, support system development and other training necessary to ameliorate life stresses resulting from the consumer’s mental illness.

13. Provide mental illness, recovery and wellness management education and training to consumer’s parents, guardians, family and/or significant others, as well as the community at large when appropriate.

14. Complete relevant documentation according to certification requirements as well as agency documentation protocols.

15. Actively participate in educational programs, team meetings, supervision meetings, and develop personal and professional goals at annual evaluations. Use supervision to build professional knowledge and skills which in turn will enhance service provision and outcomes.

16. Maintain current knowledge of agency policies/ procedures and abide by state/ federal laws.

17. Conduct self and provide services to consumers in an ethical, professional, and respectful manner while being mindful of client rights, confidentiality, and the values of a professional workplace.

18. Maintain productivity requirements.

The above listed responsibilities are not intended to be all inclusive and do not exclude any other functions customarily performed by a Case Manager Clinician which may from time to time be requested by the immediate supervisor or CEO. Work schedules may vary according to the need of the agency.

Work Environment: Case Management is a service comprised of individualized mental health activities which are delivered in a variety of locations based upon the natural environment(s) of the individual client, i.e., home and community locations.

Physical Demands: While performing the essential duties of this job, an employee is regularly required to move about; communicate, convey and engage in conversation/discussion; and operate standard office equipment; write /create documentation; detect, recognize, perceive, and observe.
