Licensed Vocational Nurse

This Position is on the South side of San Antonio located at:

Job Location: La Mision Family Health Care Clinic, 19780 South US Hwy 281, San Antonio, TX 78211

Job Summary:

Responsible for nursing care in the Clinic under the supervision of Clinic Manager and the Lead LVN. Works collaboratively with the Clinic Manager and Lead LVN in providing health education and maintaining a clean, safe work environment.

Responsibilities:

· Understands and Manifest the Philosophy and Mission of Ascension DePaul Services.

· Appointments are scheduled for patients that come through secure messages.

· Completion of Medical Records requests.

· Performs initial Client assessment (history, vital signs, current medication immunization status, drugs allergies, etc.)

· Administration of Vaccines for Children.

· Performs special tests, as needed, such as EKG and other monitor laboratory procedures.

· Provides nursing care as qualified. Refers questions to Lead LVN in the Clinic Manager's absence, to Medical provider.

· Assists in providing Health education for Individuals and groups both in the Clinic and home setting.

· Maintains Clinic area in functional order.

· Assists in obtaining lab studies from outside laboratories and follow-up client information from outside agencies, physicians and hospitals.

· Provides for networking with other agencies and refers as necessary.

· Assists with Clinic Record keeping.

· Maintains and updates professional development in related areas to primary ambulatory care.

· Applies compresses, cold packs, hot packs.

· Observes patients and reports adverse reactions to medication or treatment to medical personnel in charge.

· Collets samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples.

· Maintaining of supplies for the clinic and the pharmacy.

· Sterilizes equipment and supplies.

· Inventories and requisitions supplies.

· VFC Primary Coordinator and/or Secondary

· Ordering Supplies.

· Other duties as assigned

Complexity of Work:

• Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

Personal Protective Equipment:

• Follows Standard Precautions using personal protective equipment as required.

Education/Minimum Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. • Graduate from an Accredited School of Nursing.
• Excellent communication skills.
• Experience required in a clinic setting.
• Minimum of 1 - 3 years experience as a LVN.
• Bilingual preferred. (English/Spanish)

Licensure/Certification/ Registration:

• Current LVN license from the State of Texas required.
• Current C.P.R. Certification required.
