SUMMARY
The purpose of the Community Support Specialist I position is to act as a stabilizer and provide assessment, counseling and service linkage to youth at risk for running away, being beyond the control of their parents and/or from becoming involved in the juvenile justice system. This position provides individual, family and/or group therapy, school advocacy, life skills training and educational & therapeutic group activities as defined by client need to 10-17-year-old youth within the program service area.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Include the following and other duties may be assigned as needed.
Daily Job Duties:
Respond to all emails, texts, and phone calls from Program Coordinator within a 15-minute time frame.- Ensure the Outlook calendar updated with daily tasks and/or travel in real time.
- Maintain an active caseload of 20-25 with a maximum of 15 individual participants of higher risk clients (trauma, suicide risk, mental health, etc.) as assigned by Program Coordinator.
- Facilitate groups for discretionary clients within designated LANs
- Ensure all required crisis intervention and/or intake documentation is complete and accurate.
- Responsible for maintaining active CCBYS client files with documents not exceeding more than 2 weeks past due
- Responsible for updating and maintaining their designated LAN client, MRAI/referral, outreach, and quarterly success stories spreadsheet.
- Monitor and supervise clients to ensure their safety and well-being during programming hours and transport.
Other Duties:
Assist the Community Liaison with the upkeep and development of community relationships- Assist the Community Liaison with planning, budgeting, and executing monthly client baskets and events, as well as, delivering to clients or other LAN’s; when necessary or assigned by Program Coordinator.
- Assist the Community Liaison in planning and executing CCBYS all staff events and assigned lunch and learn presentations; when necessary or assigned by Program Coordinator.
- Assist the Community Liaison with delivering files to the correct LAN or Program Coordinator for monthly division audits and level audits; when necessary or assigned by Program Coordinator.
- Assist the Community Liaison with in coordinating and attend annual community outreach meetings; when necessary or assigned by Program Coordinator.
- Obtain Letters of Support (LOS) and Memorandums of Understanding (MOU) from community partners annually.
- Participate in individual, group supervision, team meetings, program staff meetings, and trainings as scheduled to address client issues and revised documentation, placement options, crisis schedules, etc.
- Provide required documentation (i.e. mileage, time sheets, CQI’s, surveys, etc.) as indicated by agency protocols.
- Complete all necessary paperwork and documentation, i.e. timesheets, vouchers, progress notes, incident reports, etc. and submit in a timely manner.
- Other Duties as assigned by Program Coordinator, Program Manager, and/or Program Director
Crisis Response Duties (As needed/When Assigned):
Serve as a backup responder for designated or other assigned Local Area Network (LAN), which includes: Vermilion, Kankakee, South Cook, West Cook, Will/Grundy and Kane/Kendall counties; when necessary or assigned by Program Coordinator.- Provide day-time crisis intervention/case management, as assigned, to all youth and families on a no-decline basis.
- Adheres to crisis responding schedule when assigned, by ensuring that answering service can contact worker immediately via cell phone
- When serving as a responder, will provide crisis intervention services to referring source (hospital, school, police department) via phone w/in 15 minutes and on site within 60 (urban) 90 (rural) minutes of crisis call.
- Complete all funder and/or agency required intake/crisis documents, assessments, consents, service plans and forms (Youth Assessment Screening Instrument (YASI), safety screens, internal/external referrals, progress notes and other required paperwork) within required timeframes.
- Contact Program Coordinator to obtain authorization prior to placement of any youth.
- Provide and arrange for reunification, family generated placement, or agency arranged placement at time of crisis, with a short term crisis plan, as well as, provide linkage and referral services specific to client and family needs during time of crisis.
- Ensure all required crisis intervention documentation is complete and accurate. Provide all crisis intake documents, and Telephone Logs to the designated Program Coordinator by 10:00am of the next business day
- Enter the participant into eCornerstone and the LAN specific spreadsheet within 72 hours of crisis or intake
- Work collaboratively with Community Liaison to follow-up within 24 hours on youth in shelter placement, referrals, and Minors Requiring Authoritative Intervention (MRAI) designated Local Area Network (LAN) area and update the Program Coordinator; when necessary or assigned by Program Coordinator.
- Follow-up within 24 hours on youth in shelter placement, referrals, and Minors Requiring Authoritative Intervention (MRAI) designated Local Area Network (LAN) area and update the Program Coordinator as assigned.
- Provide transportation for clients while they are in shelter or emergency placement to scheduled appointments, and school as needed.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Bachelor's degree (BA/BS/BSW) required. Previous experience with at-risk youth preferred.
