BAYADA Home Health Care is seeking a remote Clinical Support Specialist (RN required) for our Home Health Senior Care team.
The Clinical Support Specialist is a key member of the clinical quality team, responsible for reviewing and ensuring the accuracy, timeliness, and compliance of Start of Care (SOC), Recertification (Recert), and Discharge OASIS documentation. This role focuses on the 485 review, scrubbing of timely initiation of care OASIS questions, medication review, and providing targeted education to clinicians and clinical managers.
This position requires a strong background in home health operations, compliance, and documentation review. Experience as a Clinical Manager or OASIS Reviewer is required. Proficiency in Homecare Homebase (HCHB) is a plus.

Key Responsibilities
- Review and validate the 485 for all SOC and Recert assessments, ensuring accuracy and compliance with Medicare and agency policies.
- Scrub and validate timely initiation of care OASIS items, ensuring adherence to CMS guidelines and agency standards.
- Conduct medication reviews to ensure accurate medication reconciliation and proper documentation.
- Perform full review of Discharge OASIS to ensure completeness and compliance.
- Provide targeted education to clinicians and clinical managers regarding OASIS best practices, common documentation errors, and compliance requirements.
- Communicate with clinicians and leadership to address documentation discrepancies and provide recommendations for improvement.
- Assist in identifying trends in documentation errors and contribute to process improvements.
- Maintain knowledge of Medicare home health regulations, OASIS guidance, and PDGM requirements to ensure best practices are followed.
Qualifications
- Required: Minimum of 3-5 years of experience in home health, with experience as a Clinical Manager or OASIS Reviewer.
- Strong knowledge of Medicare home health regulations, OASIS-E, and PDGM.
- Experience with 485 review, medication reconciliation, and documentation compliance.
- Excellent analytical skills with attention to detail.
- Strong communication and education skills, with the ability to provide feedback to clinicians and managers.
- Preferred: Experience with Homecare Homebase (HCHB) or other EMRs used in home health.
- License: Active RN required. Must have compact license or ability to obtain license in state hired for.
