The primary objective of the Registered Nurse (RN) Case Manager position is to oversee the management, coordination, and treatment of Baywood's clients with complex or chronic medical issues. This role plays a crucial part in guaranteeing that clients receive the best and most timely healthcare services and support. By working closely with the office staff, healthcare teams, clients, and their families, RN Case Managers create and implement thorough care plans, arrange appointments and services, foster communication among healthcare providers, and diligently track client advancements.
