Comprehensive Health Assessment Professional
The role of the Comprehensive Health Assessment Professional involves delivering thorough health assessments and preventive care solutions to patients. The candidate will conduct in-home comprehensive assessments that are compliant with the annual wellness visit for Medicare, Medicaid, and Commercial Marketplace members.
This position requires a detailed evaluation of the medical and social assessment of the beneficiary members. The professional will identify and refer members/patients for routine, urgent, and emergent case management, make recommendations for follow-up with primary care physicians, and complete comprehensive assessments face-to-face in the home setting.
Requirements
- A minimum of 2 years of hands-on experience as a Nurse Practitioner
- Registration with Medicare and/or Medicaid
- Current, unrestricted license in the state of assignment
- License/certification: APRN, Certified Nurse Practitioner
- Knowledge of the HCC model, HEDIS, and STAR quality measures
- Effective communication with elderly and chronically ill members
- Proficient in the use of computer, tablet, and mobile technology
- Ability to document within available EMR
- Experience in Geriatrics, Internal Medicine, or Primary Care
