- Facilitates, implements, and evaluates the Patient Plan of Care and outcomes.
- Collaborates with the health care team to promote positive patient outcomes. Is seen as a patient advocate. Timely implementation of nursing interventions.
- Technical Skills: demonstrates appropriate clinical skills and technical competence in providing patient care. Critical Thinking: demonstrates appropriate clinical judgment. Assesses, analyzes and communicates patient information in a timely manner.
- Documentation: Accurately and promptly documents all patient assessments, interventions , responses and provider communication. Assess patient safety risk factors and identifies those patients requiring specific safety interventions including skin integrity, fall risk & appropriate use of restraints. Follows medication safety policy. Promptly initiates appropriate interventions for safety needs. Completes Safety Tracking Tools as required.
- Delegation and supervision: supervises assigned staff, delegates appropriately, takes responsibility for all care provided to patients assigned. Reports all performance issues of team members to ensure all patients receive appropriate care and attention.
- Educational needs of patient: Assesses educational needs of patient and family, provides appropriate written and verbal instructions. Documents assessment and education provided.
- Provider orders: Performs ongoing review, initiation and implementation of patient orders in a timely manner every shift.
- Prioritization: prioritizes responsibilities and issues, taking into account urgency, patient needs, and seeks appropriate resources. Problem solves independently. Multitasks efficiently and demonstrates effective time management skills.
- Professional Accountability: Independently seeks out other clinical experiences to enhance knowledge base. Actively assists others and accepts additional duties or assignments.
- Timely completion of assigned education and annual competencies.
- Actively engaged in unit based councils and performance improvement.
- Recognizes unit needs and works to help facilitate solutions.
Qualifications
- Bachelors degree in nursing preferred.
- 2 years nursing experience caring for Step Down level of care patients including cardiothoracic surgery patients, preferred.
- Ability to perform CPR. ACLS and BLS certification. NIH certification.
- PCCN certification within 2 years of start date.
- Clinical Ladder within 2 years of start date.
- Cardiac Surgery Certification (CSC) within 3 years of start date. CSU- ALS Certification within 3 years of start date.
