Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will be responsible for aquiring clients to provide support and guidance to clients facing various challenges, including behavioral health issues, developmental disabilities, and social welfare needs. This role requires the ability to work collaboratively with case managers, SSAs, guardians and clients to develop effective treatment plans and facilitate access to necessary care.
- Offer personalized care and support for individuals with developmental disabilites.
- Collaborate with healthcare providers, social workers, and community organizations to coordinate services for clients.
- Maintain accurate documentation of client progress and interventions in compliance with regulatory standards.
Qualifications
- Proven experience in behavioral health and, developmental disabilities.
- This is a sales-oriented position.
- Strong understanding of social work principles and practices, including DODD and welfare regulations.
- Excellent communication skills with the ability to build rapport with diverse populations.
- Knowledge of community resources available for clients facing various challenges.
- Ability to work independently as well as part of a multidisciplinary team.
