Job Summary
Transitions Supportive Care (TSC) is seeking a compassionate RN Case Manager to support our palliative care team. You will coordinate care for chronically ill patients, providing telephonic support to patients, families, and clinicians. This role includes clinical and administrative responsibilities to ensure comprehensive patient care.
Key Responsibilities
- Provide telephonic care coordination and triage for patients and families.
- Assess patient needs, create and revise care plans, and manage medication.
- Coordinate home health, DME, and specialist services.
- Participate in interdisciplinary team meetings and on-call rotations.
- Document patient interactions and communicate with the care team.
Qualifications
- Registered Nurse (RN) license in Florida.
- Experience in palliative or supportive care preferred.
- Strong communication, problem-solving, and organizational skills.
- CPR certification required.
Physical Requirements
- Ability to walk, stand, and lift up to 15 pounds.
- Must be able to drive in varying conditions and access different facilities.
