OVERVIEW:
We are seeking a dedicated and compassionate Case Manager to join our team. This role involves overseeing and supporting mentor-mentee relationships, ensuring the success and impact of our mentoring programs. The ideal candidate will have experience in case management, a strong commitment to youth development, and excellent communication skills.
RESPONSIBILITIES
Match Coordination:
- Facilitate the matching process between mentors and mentees, considering interests, backgrounds, and program goals.
- Conduct thorough assessments of potential mentors and mentees.
Support and Supervision:
- Provide ongoing support and guidance to mentor-mentee pairs, addressing any challenges or concerns.
- Conduct regular check-ins and progress evaluations with both mentors and mentees.
Training and Development:
- Develop and deliver training programs for new mentors and provide ongoing training opportunities.
- Organize workshops and events to enhance the mentoring experience.
Case Management:
- Maintain accurate and up-to-date records of all mentor-mentee interactions.
- Ensure compliance with program policies and procedures.
- Monitor and report on the progress and outcomes of mentoring relationships.
Community Outreach:
- Build and maintain relationships with community partners and stakeholders.
- Promote the mentoring program within the community to recruit new mentors and mentees.
Evaluation and Improvement:
- Conduct regular evaluations of the mentoring program’s effectiveness.
- Use feedback to continuously improve program quality and impact.
QUALIFICATIONS:
- Bachelor’s degree in social work, psychology, education, or a related field (Master’s preferred).
- Minimum of 3 years of experience in case management, youth development, or a related area.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Experience working with diverse populations.
- Proficiency in Microsoft Office and case management software.
- Valid driver’s license and reliable transportation
Job Type: Full-time
Pay: $36,000.00 - $38,500.00 per year
