Title: Rapid Rehousing Case Manager CLMCAA

Reports to: Housing Administrator

Job Purpose

The Rapid Re-Housing Resource Specialist position assists households (single individuals and families) transition from Homelessness to Housed. This position is responsible for helping households identify their barriers of stable housing through financial assistance, long-term Case Management, and referrals (internal and external agency).

Duties and Responsibilities

· Implement agency defined assessment processes and follow defined protocols to serve clients through the appropriate service track within the utility and housing programs.

· Implement the current procedure/plan that addresses each households’ needs, and upon approval, assist the clients by engaging them toward self-sufficiency.

· Routinely evaluate the progress toward goals identified in each plan of service and recommend plan modification as needed to encourage ongoing progress toward self- sufficiency and defined program outcomes. Assessments completed at move-in, monthly, and annually; SPDATs at move-in, 2 weeks after move-in, 30 days, 60 days, 3 months, 6 months, 9 months, and annually. After the 1st year SPDATs are to be completed annually.

· Recommend resident referral to suitable resources and agency partners. Use problem-solving skills to navigate household barriers to self-sufficiency.

· Coordinate with appropriate agencies for referrals: CMH, Tribal, Veteran, Housing, Child Services/Needs, Food, Healthcare, Life Skills, Social Security, Disability, etc.

· Develop a professional and supportive relationship with each resident that establishes trust and serves to motivate toward self-sufficiency.

· Advocate on behalf of residents and resident rights at local, county-wide, and state platforms.

· Network with community-based programs and agencies to bring in services, programs, and resources to the residents.

· Use skills and experience to develop and implement housing and service plans within program criteria and grant guidelines. Utilize MSHMIS data base to create housing plan(s), goal(s), etc. All data (assessments, interims, client interactions, services provided, needs identified, case notes, etc.) is to be entered in HMIS immediately. Assessments should be completed live as much as possible.

· Use established reports from HMIS (Service Point) and/or other relational databases to recommend continuous quality improvement processes to improve program outcomes.

· Run/use established reports (HMIS Data Quality, CSV, APR, CoC, etc.) from HMIS (Service Point) and/or other relational databases to recommend continuous quality improvement processes to improve program outcomes.

· Be thoroughly familiar with approved assistance grants, their conditions, and eligibility criteria.

· Work closely with other staff members to ensure data quality.

· Create reports, business correspondence and procedure manuals as assigned.

· Travel independently to areas served by C-L-M Community Action Agency, with appropriate compensation.

· Meet with Housing Administrator as scheduled to review files for service plan coordination, organization, etc.

· Network within the agency and out in the community.

· Attend staff meetings and Local Planning Board Meetings to identify services within the community.

· Occasional planning of fun events for residents.

· Identify local units available for rent

· Maintain a current list of landlords

· Build/maintain relationship with landlords

· Other duties as assigned.

DHHS Consolidated Rapid ReHousing Grant:

· Knowledge of grant guidelines, program manual, policy, procedure, and eligibility requirements.

· Continual education to make sure all updates are being implemented and followed.

· Creation of tools and resources/packets to assist households and landlords understand the program, exit successfully.

MSHDA Housing Choice Voucher Homeless Preference Waitlist:

· This position will be required to gain access as a Key Person by signing a Key Person Security Agreement with MSHDA.

· As the Lead Agency for MSHDA, this position would be responsible for assisting homeless/at-risk households get onto the MSHDA HCV Waitlist with Homeless Preference for the Tri-County Area (Chippewa, Luce, Mackinac).

· This position will be required to travel to multiple satellite offices to assist households within the tri-county service area get placed on the MSHDA HCV HP waitlist.

· Work effectively with MSHDA Housing Agent(s)

· Help maintain the wait list for the Tri-County area: adding clients to wait list, recertifying households at 90-120 days, etc.

DHHS MI Bridges Navigator:

· This position is required to complete all training needed to become a DHHS Navigator.

· This position will be required to assist with completing a MI Bridges application for program clients.

· This position will be required to assist client retrieve key DHHS documents as well as update/scan in documentation needed for DHHS services.

· Ability to provide assessment, case management, and follow up services to residents as directed.

· Ability to relate to people on a one on one basis and as a member of a group.

· Ability to effectively communicate with distressed persons under adverse conditions.

· Ability to work as part of team focused on achieving well defined performance outcomes.

· A working knowledge of the barriers and challenges for homeless and/or low-income households.

· Ability to exercise judgment and initiative.

· Strong familiarity and skill with data entry, computer systems, computer software programs (Microsoft Excel, Word, PowerPoint, Outlook, etc.).

· Strong organizational, and office skills.

· Ability to multi-task and meet important deadlines with strong attention to details.

· Ability to problem solve, exercise good discretion and judgment when making decisions.

· Possess and demonstrate skill and experience in effectively coordinating multiple projects with short deadlines.

· Ability to work with a variety of systems, agencies, data and individuals while exhibiting excellent communication skills.

· Ability to maintain records and prepare verbal and written reports.

· Ability to explain program policies and procedures to consumers and families of consumers.

· Ability to establish and maintain effective working relationships with administrators, employees, consumers, and families of consumers.

  • Absolute ability to maintain consumer and agency confidentiality.

Integrity

· Ensure confidentiality of clients, agency and co-workers

· Understand and promote the goals and philosophy of CLMCAA.

· Become familiar with CLM CAA’s Employee Handbook and follow policies outlined therein.

· Has general knowledge of and adheres to the Agency’s mission statement.

· Adhere to CLM CAA policies and procedures.

· Represent CLM CAA, its policies and point of view while interacting with other agencies and individuals.

· Works the number of hours designated in personnel action, reports absences to immediate supervisor at least one hour prior to scheduled time to report.

· Reports to work area ready to begin work at the assigned time.

Required Qualifications

High School Diploma or equivalent required.

Bachelor’s degree preferably in social work, human services, community development, business, or a related field. Or the equivalent education and experience in the human service field. Prior experience working in the human services field preferred.

Will be required to pass criminal history check.

Must have valid Michigan driver’s license, an acceptable driving record and reliable transportation.
