Bloom is seeking an exceptional Behavioral Health Consultant to join the team! This position will be joining Bloom’s Behavioral Health team, with a focus on caring for Bloom’s geriatric patients with underlying psychiatric conditions.
Behavioral Health Consultant Key Responsibilities
- Mental health assessments, including administration of validated scales
- Provide lifestyle coaching to address anxiety, depression, sleep disturbances, and weight loss
- Use multiple modalities to engage with clients like motivational interviewing, problem solving therapy, behavioral activation, and provide grief support
- Coach caregivers and family members to support and advocate for patients
- Monitor symptom severity, treatment adherence and side effects
- Track client progress through the use of a registry
- Report results to primary medical provider and/or the consulting psychiatric consultant
- Identify social service needs and refer where appropriate
- Participation in weekly caseload review with psychiatric consultant
- Ongoing collaboration and coordination with treating providers
- Relapse prevention planning and preparation for discharge from active treatment
- Mostly live care management in client’s homes or assisted living communities, but some virtual visits may be warranted
Behavioral Health Consultant Qualifications
- RN with experience or high interest in the mental health space
- LCSW with experience or high interest in the mental health space
