Essential Job Functions:
Provide case management services for ES residents
Facilitate and complete discharge plans and discharge summaries
Request placement variances TDFPS on ES residents
Represent Boysville at court hearings of ES residents
Communication with CPS staffing if needed.
Coordinate ES resident needs (haircut, hair care products, clothing, sibling contact)
Coordinate visitation and ensure children are present for scheduled visits
Develop the resident file for placement in the residential program
Provide aftercare follow-up contacts.
Be in continued contact with caseworker at least once a week. The information that needs to be provided includes any incidents, placements and any other shelter responsibilities.
Plans of discharge should be individualized and done reguarly and on time.
On call for Emergency Shelter. Be available both on office phone and cell phone.
Will need to build relationships with all ES staff including Child Care Specialist.
Advocate for the children’s cultural and day to day needs. Make sure that all the children’s needs in all areas are being addressed and taken care of consistently.
Provide more information in the assessment meetings, while talking at a slower pace.
Follow-ups on incident reports. Report all incidents to caseworker regarding the day to day care and involvement of the child weekly or as needed.
Update and complete all files
Have the child’s signature of Plan for Discharge Summary. (The child is being informed or involved with the discharge plan).
Email or fax completed discharge form to the caseworker before the deadline.
Qualifications
Must have a Bachelor’s Degree in Social Work or related Human Services field. Licensed Bachelor Social Worker and 1 to 2 years experience in a similar position, preferred. Must possess excellent communication, organization, time management and written skills. Must be computer literate in windows applications. Must exhibit behavior that demonstrates cultural diversity and sensitivity (See Corporate Policy #2103).
Physical/Mental Requirements:
Ability to read, write and converse in English. Ability to communicate with staff, residents, families, vendors, support agencies and others. Ability to handle stress. Bending, lifting, grasping, fine hand/eye coordination, pushing/pulling, prolonged sitting/standing. Ability to respond to telephones and other auditory stimulation. Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).

We offer a competitive base salary range of $38,000 - $40,000 per year depending on experience.
