MinT Case Manager

Description

Summary:

Responsible for providing case management services. Work schedule will reflect meeting the requirements under the essential job functions and will be overseen by the MinT Director. All employees of Boysville, Inc. receive training in the Quality Improvement (QI) activities of the company and are required to actively participate in QI meetings, tasks, projects and/or committees, as needed.

Essential Job Functions:

• Provide case management services for MinT (Mother's in Training)residents
• Complete social histories involving initial intake interview with child and guardian(s) for consideration of long-term residency
• Complete monthly chronological records
• Maintain a complete computer and paper file on each child in case load
• Attend regular ARDs and court hearings
• Complete regular six-month Plans of Service involving child assessments
• Complete Quarterly Reviews
• Consult and support Child Care Staff with the above services
• Facilitate purchasing of clothing for residents
• Oversee arrangements for special needs of children during holidays or other special events
• Participate in Administrator on Duty program
• Attend Interdisciplinary Team Meetings and Assessment Team Meetings
• Attend Administrative Staff Meetings
• Serve on PQI Records Review Committee
• Make relevant referrals
• Coordinate and facilitate timely implementation of discharge plans
• Maintain continuing communication with children, foster parents, parents, TDFPS caseworkers, attorneys, CASA volunteers and others involved in case planning
• Respond to emergencies when needed
• Act as a liaison between Boysville and TDFPS

Qualifications

Qualifications:

• Must have a Bachelors Degree in Social Work or related Human Services field.
  • Licensed Bachelor Social Worker and 1 to 2 years experience in a similar position, preferred.
• Must possess excellent communication, organization, time management and written skills.
• Must be computer literate in windows applications.
• Must exhibit behavior that demonstrates cultural diversity and sensitivity (See Corporate Policy #2103).

Physical/Mental Requirements:

• Ability to read, write and converse in English
• Ability to communicate with staff, residents, families, vendors, support agencies and others
• Ability to handle stress
• Bending, lifting, grasping, fine hand/eye coordination, pushing/pulling, prolonged sitting/standing
• Ability to respond to telephones and other auditory stimulation.
• Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).

Salary : $34,200 - $43,400
