TITLE: Case Manager
JOB PURPOSE: The Case Manager is responsible for facilitating the achievement of client wellness and independence through advocacy, assessment, planning, communication, education, resource management, and service facilitation, based on the individual needs of the clients within the Bread of Life shelter program.
GENERAL EXPECTATIONS:
A) Be committed to the Bread of Life mission
B) Work as a member of a team in the performance of duties
C) Be punctual for scheduled work and remain alert at all times, sleeping or the appearance of sleeping is strictly prohibited
D) Work in harmonious relationships with all vendors and members of the community
E) Perform duties in a conscientious, cooperative manner
F) Keep work area neat/clean and maintain a professional appearance
G) Be thoroughly knowledgeable of all Bread of Life policies and procedures
H) To accept shared responsibility with other employees to successfully accomplish common goals
I) To assure quality in work performed in order to facilitate the delivery of quality services
J) Maintain confidence and protect Bread of Life by keeping information concerning clients and shelter operations confidential
ESSENTIAL JOB RESPONSIBILITIES:
1) Evaluate, facilitate and monitor services and clients’ progress in relationship to established goals and objectives. Document daily in the Client Record
2) Plan, organize, and monitor Plan of care follow-up, services to residents, and over-all conditions in the shelter
3) Maintain files in an organized, confidential manner following policy and procedures and state and federal guidelines
4) Facilitates community resource availability based on client needs to assure maximum benefit to the clients
5) Knowledgeable in use of office equipment used in the execution of job duties; must be able to utilize this equipment with or without reasonable accommodation
6) Excellent organization and verbal and written communication skills
7) Respond to public telephone inquiries. (Media inquiries should always be directed to the Executive Director)
8) Participates in Agency-wide training/in-services programs
9) Participates in interdisciplinary treatment planning meetings, works with the shelter staff to make necessary improvements
10) Maintain relationships and work closely with local and state housing authorities
11) Able to work independently in a fast-paced environment, demonstrating good judgment
12) Presents self professionally, positively representing the organization in demeanor and appearance
13) Individual may perform other duties as assigned including work in other areas to cover absences or relief to equalize peak work periods or otherwise balance the workload
REQUIREMENTS AND SKILLS NEEDED FOR POSITION:
· A Bachelor’s degree in the human services field preferred, or the equivalent in years and experience
· Must possess a valid Driver’s License and maintain a clean driving record
· Ability to communicate effectively orally and in writing
· Experience working with people with diverse backgrounds including persons with disabilities, behavioral health issues, and families with children
