Overview:
As a Case Manager, you will ensure seamless patient care from admission through discharge by assessing, planning, implementing, coordinating, monitoring, and evaluating patient caseloads. You will serve as an advanced clinical resource for patients, families, and staff, working collaboratively with the interdisciplinary team to provide comprehensive, cost-effective care. Continuous improvement of patient outcomes is a key responsibility.
Qualifications:
- BSN required for all new hires starting from March 31, 2014; Master's degree preferred
- Minimum of 3 years of experience in an acute care hospital setting
- Licensed Registered Nurse in good standing with the State of Michigan
- Case Management Certification preferred
- Proficiency in word processing, spreadsheet, and presentation software
- Ability to handle multiple priorities in a stressful environment
- Strong verbal and written communication skills
- Effective negotiation and conflict resolution skills
- Demonstrates critical thinking, decision-making, and problem-solving abilities
- Independent, self-motivated, and capable of managing multiple demands
Responsibilities:
- Conduct early assessments to identify patients at risk for post-hospitalization care and services.
- Perform further assessments and develop patient transition care plans with the care coordination team.
- Monitor and document patient progress, providing ongoing assessment and daily rounds.
- Identify readmissions and necessary interventions to prevent further readmissions, communicating plans to the multidisciplinary team.
- Coordinate care transitions and ensure efficient handoffs between services.
- Document clear and specific transitional plans that meet patient needs and preferences.
- Work with the community to identify and allocate post-discharge needs.
- Make appropriate referrals to extended care resources and community services.
- Act as a liaison between patients, physicians, ancillary services, and community services to ensure effective healthcare management.
- Develop, implement, and communicate the plan of care from acute phase through transition out of acute care.
- Build and maintain strong relationships with physicians and the nursing team.
- Participate actively in assigned groups and committees.
- Ensure the appropriate use of community and outpatient resources.
- Manage and coordinate discharge plans to ensure appropriate length of stay and facilitate quality, clinical, and financial outcomes.
- Evaluate outcomes related to the case management process, including length of stay, readmission reports, patient satisfaction, and financial variances.
