What are the responsibilities and job description for the Clinic Social Worker position at Cabell Huntington Hospital/St. Mary?
The Clinic Social Worker (CSW) exercises independent judgment and professional discretion in the practice of case management to a selected group of patients. The CSW collaborates with the Mind Wellness Team and serves as advocate to help patients and families navigate care. The CSW is proactive and uses collaborative problem solving skills to meet the needs of the patients and Providers. The CSW provides social work case management services as needed and relies heavily on such resources as family, community, state and federal agencies as well as the patient's resources to assist the patient's and/or family's return to a productive life.
Requirements:- Bachelor's degree is required
- Master's degree is preferred
- Social work licensure in the state of WV: LSW is required
- Minimum of up to 1 Year of Position-Related Experience is required
- 2 to 5 Years of Position-Related Experience is preferred

Social work experience is required and this experience in the medical field is preferred. The learning period recommended is six (6) months for proficiency in the majority of tasks.
