CareOne Home Health is looking for a Per Diem LVN for our growing Home Health business serving Stanislaus and San Joaquin County. Mileage is reimbursed and driving time is paid.

Primary responsibilities will be:

• Serving as a point of contact throughout the admissions process
• Ensuring coordination of care with other team members and marketing team
• Communicating effectively with patients, family members, case managers, healthcare providers and co-workers
• Providing education to patients and family members regarding their home healthcare experience
• Providing an outstanding admissions experience for our patients

Requirements:

• 2 years LVN experience, home health preferred
• LVN license
• Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• Experience with Kinnser (WellSky) EMR preferred
• Experience with Allscripts preferred
