Description
The ESSHI Case Manager is responsible for supporting clients in maintaining stable housing. The position provides support to community members who have experienced episodes of homelessness and works to ensure clients have access to high quality support services. Activities conducted by this position fall within the following three categories: 1) Care coordination and support service referrals 2) Individualized service plan development and review and 3) Administrative program oversight and regulation compliance.
Requirements
ESSENTIAL RESPONSIBILITIES
- Determine if client referrals meet eligibility guidelines for the housing program.
- Provide services directly to clients or determines with supervisor if a referral to a partner agency is more appropriate.
- Responsible for scheduling and facilitating monthly meetings with medical providers to coordinate and track mutual client services.
- Track important client medical metrics.
- Support clients to engage and receive medical care.
- Support clients to find and maintain stable housing.
- Responsible for regularly updating client information within a secure database.
EXPERIENCE
- At least one year of direct case management or relevant experience.
EDUCATION
- Bachelor’s degree preferred.

TRAVEL
- Must have a clean and valid NYS license.
- Travel in the field as necessary.
LOCATION
Albany, NY
WORK WEEK
Full time role, 40 hours per week. Monday through Friday with hours flexible between 7:30am-6:00pm
Salary : $25 - $26
