What are the responsibilities and job description for the Public Health Nurse (CMHRP) position at Catawba County Government?
Catawba County Public Health is recruiting, for a highly motivated, energetic Public Health Nurse to provide nursing/case management services to eligible pregnant women with priority risk factors in order to improve pregnancy outcomes. In this position, you will promote the receipt of comprehensive, preventative health services to pregnant women and their newborns and assist them in addressing their health, behavioral, and psychological needs in a clinical and home visiting setting.
- Identify potential clients and determine eligibility for the Care Management for High-Risk Pregnant Women (CMHRP) program
- Complete accurate and concise documentation, complete pregnancy assessment, develop plan of care, and determine level of case intensity for each client
- Conduct face to face contact with clients, either in their home, OB practice, or community
- Run and review various reports to identify potential at risk clients, monitor productivity and quality improvement efforts
- Manage a caseload of CMHRP patients residing in Catawba County
- Develop and manage schedule contacts based on identified level of intensity needed
- Provide assessments, resource linkage, education, and referrals to clients
- Conduct outreach/marketing to increase community awareness of CMHRP program through written information, meetings with physicians/others, and coordination with other service programs
- Perform departmental duties such as attending meetings, trainings, supervisor meetings, and other responsibilities as needed
- Bachelor's degree in Nursing or Associates degree from an accredited school of professional nursing and one year of professional nursing experience.
SPECIAL QUALIFICATIONS:
- Must be licensed to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
ADDITIONAL REQUIREMENTS:
- A valid Driver’s license with a safe driving record
- Current certification in Basic Life Support (CPR/BLS)
- Solid knowledge and skills in regard to case management
- Excellent communication and interpersonal skills to work well with a wide span of diverse people and professionals on all levels
- Strong assessment and referral skills
- Excellent organizational and time management skills
- Sound knowledge of community resources, experience working with pregnant women, and ability to advocate for clients
- Strong analytical and problem-solving skills
- Ability to work independently with minimal supervision
- Effective computer skills to access and document in medical records
- This is considered Light Work, requiring ability to stand, walk, and lift to 30 pounds.
