What will I do as the In-Home Case Manager?
This position provides case management services to help individuals live independently in clean, safe homes or apartments to delay institutional placement. The Case Manager works collaboratively with a team of Homemakers serving older adult clients and adults with disabilities.

What will I do as the In-Home Case Manager?
This position provides case management services to help individuals live independently in clean, safe homes or apartments to delay institutional placement. The Case Manager works collaboratively with a team of Homemakers serving older adult clients and adults with disabilities.
- Conduct the initial in-home assessments, identifying needs, and developing care plans.
- Collaborate with Homemakers to monitor progress, ensure quality of care, problem solve through challenges, providing support and guidance in accessing other services (such as Medicare, food stamps, etc.)
- Manage client and homemaker schedules and appointments to meet client needs.
- Respond to questions, complaints, and concerns of clients, families, and staff.
- Enhance the emotional well-being and social functioning of clients.

- Bachelor’s degree in social work, nursing, or related field.
- At least 6 months of experience working with older adults, people with disabilities, and a variety of physical and mental illnesses is a plus; 2 or more years is strongly desired.
- Travel is required, approximately 50% of the time. Valid driver’s license, reliable transportation and minimum liability insurance on vehicle are all required.
- Strong interpersonal skills; a team player with a passion for developing and maintaining strong relationships.
- Excellent oral and written communication skills. Able to tactfully address confidential matters efficiently and discretely.
