The Transitional Housing Homeless Outreach Worker is the first contact for many homeless youth ages 16 to 24 . The Transitional Housing Homeless Outreach Worker position is responsible for :
1. Connecting resources to youth who are homeless and providing a nonjudgmental approach to helping solve their crisis.
2. Provides case management and counseling to youth in crisis on the streets.
3. Makes decisions on qualifications for the Catholic Charities services for homeless youth housing program and the onsite transitional house.
4. Responds to referrals coming to program and makes referrals when necessary to other programs.
5. Performs screening / intake with new referrals and outreach youth, explaining services to the client and completing all required paperwork.
6. Maintains professional relationships with community resources.
