What are the responsibilities and job description for the Care coordinator position at CDAC Behavioral Healthcare?
CDAC Behavioral Healthcare is seeking a Veteran's Court Care Coordinator to coordinate and oversee court-ordered care for Veterans in the Santa Rosa Veteran's Court Program.
This is a part-time position (up to 20 hours per week)
Essential job duties and responsibilities of this position are; but not limited to :
Responsible for the day-to-day operations of the Veteran's Care Program
Attempt to contact all participants referred to the program
Assist participants during the admission process to recommend treatment programs / facility
Identify participant needs and make referrals as necessary
Attend client staffing and court hearings
Attend weekly court meetings with the judge and treatment team
Qualifications
- Bachelor's degree in Social Services field
- Minimum of two (2) years of relevant experience and knowledge of community resources
- Excellent communication and presentation skills
- Strong critical thinking and decision-making skills
- Strong organization and time management skills
- Ability to take initiative and work independently
- Ability to manage multiple job tasks with continuously changing priorities
- Experience working with at-risk and culturally diverse population
- Experience in substance abuse and / or case management
- Valid Florida driver's license and good driving record
- Ability to transport clients
- Proficiency in Word, Excel, and other computer-related tasks
- Preferred :
- Non-profit and / or social services industry experience
- Knowledge and experience in care coordination for substance misuse population
