Janian is seeking a part-time (i.e. 21 hrs/wk) licensed practical nurse (LPN) to provide supportive services for Center for Urban Community Services’ transitional housing program, located in Harlem, Manhattan. CUCS is a leading housing placement program in NYC serving individuals experiencing homelessness and mental illness. CUCS provides person-centered services, including housing placement, comprehensive health, mental health, and case management, with a harm-reduction approach. The primary goal of on-site services is permanent housing placement for program participants which meets their needs and abilities. Operating 7 days a week, the transitional housing program offers 24 hour staffing, including security and social service teams.
The Licensed Practical Nurse will work at the Harlem, Manhattan facility in collaboration with both CUCS and Janian Medical staff, which will include a primary care provider, psychiatrist, medical assistant, and social workers. Flexible Daytime or Evening shifts.
Responsibilities
The central role of the LPN is to:
1. Manage and lead the Assistance with Self-Administration of Medication (AWSAM) program. This includes managing the medications, as well as the associated documentation, communication, and coordination needed to run the program.
Additional nursing responsibilities included in the position are to:
2. Provide nursing care at the order of a Janian provider, and in some cases a non-Janian provider:
a. Observe, measure, record, and report clinical data relating to patient health status.
b. Administer injections, such as: Long Acting Injectable (LAI) medications, Tuberculin Skin Test (TST) implantations, and vaccines.
c. Assist patients with wound care.
d. Support client health behaviors and activities, including personal hygiene assistance.
e. Provide health support and teaching to clients, providing one-on-one counselling and running educational groups.
f. Maintain a patient-centered, harm reduction approach to patient interactions.
3. Support the primary care and psychiatry programs at the site, and work collaboratively with the site social work staff to:
a. Escort patients to healthcare appointments.
b. Compile patient health information by communicating with outside providers to coordinate care and obtain patient health information, reports, and consult notes.
c. Maintain medical office equipment and complete inventory of supplies, as needed.
d. Participate in staff meetings and other case conferencing meetings as scheduled.
e. Maintain a professional, working relationship with site staff.
