Janian’s primary care program, started in 2012, continues to expand rapidly. Janian Primary Care Providers provide quality primary care in a variety of settings including permanent supportive housing, transitional shelters, safe havens, drop-in centers, as well as within the community through Street Medicine. Primary Care Providers work closely with supportive staff as well as collaborate with behavioral health providers, social workers and case managers at program sites and residences, and street outreach teams who identify and refer medically vulnerable clients.
