Job Description:
Summary of Responsibility
In keeping with the Mission/Vision/Values of Centerstone, a Health Home Clinician is responsible for providing supports and services to adult clients and their families primarily in the client’s natural environment to facilitate the achievement of personal growth goals. This involves providing individual and group therapeutic interventions, using an integrated health approach, that facilitate personal growth, skill building, identification and use of natural supports, and use of community resources. The Health Home Clinician is also responsible for assisting with assessment and personal growth planning, linking clients to appropriate services, advocating on their behalf, providing support, monitoring effectiveness of services and providing crisis services as needed. The Health Home Clinician is required to use independent discretion and judgment when providing services to clients. As part of Centerstone’s team-oriented philosophy, the Health Home Clinician is expected to actively participate in a variety of cross-functional teams and performance improvement initiatives.
ESSENTIAL DUTIES
Completes job requirements with independent action and resourcefulness and seeks out additional duties during periods of downtime. Organizes time, effort, and use of resources to achieve desired outcomes.

This also includes consistently returning phone calls and e-mails within 24 hours and updating voicemail/out of office e-mail, as well as consistently using pleasant tone and having a helpful attitude.
Assist clients with developing their Wellness Recovery Action Plan (WRAP). Provide wellness consultation. Work in consultation/collaboration with clients, family members and other providers to implement plan.
Provides assertive outreach, advocacy, support and follow-up to assigned clients. Documents all outreach attempts and ensures all services/ outreaches indicated in the personal growth plan are provided in accordance with identified frequency.
Provide ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.
Assist clients with specific, identified physical health conditions and needs to connect and collaborate with primary care, specialty care & other care delivery systems, provide coaching to promote understanding of disease states and how to self-manage symptoms, reduce the impact of key illnesses that result in shortened life expectancy and quality of life, encourage better health habits through education, directly address basic physical health care needs in conjunction with psychiatric care, enrich services to focus on whole health, otherwise support each client on their journey to improved health goals.
Provide transportation assistance to help clients achieve and maintain wellness goals and initiatives.
Meet or exceed Key Performance Indicators (KPI’s) expectations.

Completes all required documentation legibly, accurately, timely, and thoroughly using clinically appropriate terminology and containing clinically relevant information and applicable standards. Demonstrates utilization and incorporation of audit feedback when completing documentation.
Supports the strategic goals and mission/vision/values of Centerstone at all times when interacting with clients, co-workers and professionals from other organizations and presents a positive and professional demeanor.
Assists client in building a natural support team. Integrates natural community supports/resources, for any and all life domain area(s) identified as needing improvement, into the personal growth planning process and service delivery process. When clinically indicated, serves as representative payee and assists client in learning budgeting and money management skills. Coordinates with primary healthcare provider and documents integrated care with physical health care provider in the clinical record.
Treatment/intervention must be recovery focused taking into account trauma informed care practices. Demonstrates the ability to help the client identify, select and practice strategies allowing the client to develop the knowledge, skills and/or attitudes required for successful relapse prevention and attainment of personal growth goals.
Presents relevant information for the purpose of assuring quality of care, gaining feedback, and planning changes in provision of personal growth-based services. Identifies and reports all Critical Incidents according to Centerstone policy. Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated. Does not leave crisis situation until client is in a safe environment or crisis is resolved.
Attend staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team. Utilizes supervisor, clinical on-call and Ethics Review team appropriately and when indicated/required.
Meet and maintain applicable certification and job requirements.
OTHER DUTIES AND RESPONSIBILITIES
Participate in training designed to enhance job-related skills. Provide community education as assigned. Perform other duties, not inconsistent with qualifications and regular duties, as assigned.

  • Knowledge of basic casework/social work practices
  • Knowledge of internal and external referral procedures
  • Knowledge of applicable state and/or federal statutes (e.g., Mental Health Code and Confidentiality Act).and others pertaining to position

RESPONSIBILITY
Supervisory Controls: The Department Manager interprets and enforces policies through direct Supervision. The Health Home Clinician maintains considerable autonomy in daily functioning, decision making, and monitoring progress of clients towards goals and objectives, and work prioritization. Majority of work is reviewed at some point.
Guidelines: Board, Administrative and program policies, licensure, accreditation, statutory and regulatory standards restrict the position’s discretion in completing job tasks. Staff has the prerogative to propose changes in practice, procedures and policies that affect their team.
DIFFICULTY
Complexity: Position is characterized by a series of both routine and complex tasks. The variety of client needs and provider requirements, as well as the number of providers with whom the Health Home Clinician must interact, requires maximum flexibility to ensure successful work performance.
B. Scope & Effect: This job affects the physical, emotional, and psychological well-being of service recipients and impacts the work of other internal and external direct service providers and support staff working with assigned target population.
PERSONAL RELATIONSHIPS
Personal Contacts
The Health Home Clinician interacts regularly with other program staff throughout the Corporation, support staff, staff from a variety of other community agencies, inpatient facility staff, and State code department staff.
Purpose
Contacts are for the purpose of staff and community provider managing service delivery among multiple providers.
ENVIRONMENTAL DEMANDS
Physical Requirements - Work is primarily sedentary, but may require some physical exertion on client outings or recreational activities.
Work Environment- Work is performed in a wide variety of community settings, including office; service recipient residences, outdoor locations, and community service agencies. Considerable travel is involved. Health Home Clinician frequently works under non-climate-controlled conditions and may be exposed to various health hazards.
MINIMUM CREDENTIALS/REQUIREMENTS
Requires a Bachelor's Degree in a human service related field and/or five (5) years of paid experience in the mental health field or combination of an internship and experience equivalent to five years of experience. Bachelor's Degree is strongly preferred. Position requires valid driver’s license, safe driving record, and experience working with the target populations, human service work, crisis intervention work preferred. Must successfully complete all required background screenings. Requires utilizing electronic health records and/or other automated/electronic documentation systems.
