Central MN Mental Health Center has opportunities for a Full-Time Care Navigator: Intake Specialist in our Buffalo Office. The Care Navigator: Intake Specialist position is an administrative job with responsibility for the intake of all new clients. This position will be responsible for administrating mental health screenings, physical health measurements, and recording required client profile fields. The Duties may include but are not limited to:
- Provide exceptional customer service;
- Meet with new clients to administer client questionnaires, screeners and enter scores into Credible;
- Schedule new client appointments and assign appropriate clinician;
- Prepare and open charts for new clients;
- Process referrals from internal and external parties
- Obtain client’s MRN # through EPIC, as needed
- Meet with new clients to review and sign client forms and information
- Obtain and verify client insurance
- Take client’s vitals
- Complete release of information
- Scan information into Electronic Health Record software
- Travel to other CMMHC locations for coverage as needed
The wage range for this position is $16.50- $17.70. The starting salary for each application will be based on years of experience for the position.
- High School Diploma or GED
- Mental Health Training or MA Preferred
- Valid Driver’s License and acceptable driving record.
- Is at least 18 years of age or older
