Aging Case Manager

JOB DESCRIPTION
Classification: Non-Exempt
Department: Area Agency on Aging
Reports to: Aging Program Director

Summary/Objective
The Case Management program consists of planning, arranging for and coordination of appropriate home and community-based services which shall be provided to clients in a timely and cost effective manner. Incumbent assigned to this position maintains considerable contact with service providers, consumers of services, civic groups and the public and is responsible for providing case management and supportive services to eligible clients.
The Case Manager provides the comprehensive assessment by which an individual's needs for services are determined, arranges for those services in an organized and coordinated way to meet goals and objectives of an individualized service plan, and periodically monitors and re-evaluate progress in the attainment of those goals.

Essential Functions

1.Prepares and maintains client files, keeps records confidential, and completes required reports.
2.Collect, verify, and compile data from various sources; maintain records as required on state and company/department databases.
3.Coordination of Services- assessing client needs, identifying the most appropriate services to meet their needs, and linking them with the agency providing the services, completing Customer Information Form (CIF) as needed.
4.Case Management – assessment of individual’s needs are determined, arrangement for those services in an organized and coordinated manner; provides a systematic process of service planning, monitoring, and follow-up for individuals who meet the requirements for the Home and Community Based Program.
5.Information, Assistance, and Referral – link and inform older individuals and their caregivers of the available opportunities, services, resources, and programs in the community through incoming calls, emails, and referrals.
6.Assists in resource development, coordination of services, and individualized care plans which services to limit duplication, assist in client targeting, and to maximize use of informal support systems based on resource availability.
7.Assists in program and care plan monitoring to ensure attainment of proper objectives and goals as outlined in Area Plan as well as appropriateness of service delivery, ensuring that all quality assurance standards are met.

Supervisory Responsibility
This position has no supervisory responsibilities.

Work Environment
While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload. The employee is occasionally exposed to chemical hazards, respiratory hazards, combative residents, and to blood pathogens and body fluids.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is regularly required to stand, walk, sit; use hands to finger, handle or feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 20 pounds. Specific vision abilities required by this job include close vision distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5:00 p.m.

Travel
Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

Required Education and Experience
Bachelor’s degree in Social Work (or related human service/social service field or health-oriented field) from an accredited college or university
AND
At least two years of full-time experience in direct services with the elderly or disabled population

Additional Eligibility Qualifications
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to establish and maintain effective working relationships with others.
  • Thorough knowledge of the principles, procedures and techniques of professional social work; knowledge of community resources available to individuals and families; knowledge and skill in interviewing and assessment techniques; and knowledge and understanding of psychological, social, health and economic factors influencing the attitudes and behavior of individuals and families, especially as they relate to the elderly.
  • Detail oriented with strong organizational skills.
  • Ability to work in team environment and initiative to work alone.
  • Must have and maintain a valid Mississippi driver's license.

Other Duties
1.Attends and or provides relevant meetings, training sessions and conferences as assigned by Director.
2.Serve as back-up and supplemental assistance for designated duties within the Aging Division.
3.Maintain Inservice training levels for designated program(s).
4.Assists with maintenance of department assigned company vehicles.
5.Assists Aging Director with the NFCSP supplemental services (i.e. Wheelchair Ramps).
6.Assists Aging Director with identifying vendors to provide seniors with minor home repairs.
7.Oversees the administrative functions of the Chore Services Program and Lawn Services.
8.Other related work as required.
